Crisis Community Stabilization

Community Stabilization services provide temporary, structured support to individuals and their natural support systems during periods of emotional or behavioral crisis.

Crisis Community Stabilization is a short-term, intensive service designed to support individuals experiencing emotional or behavioral crises. These services are often provided following contact with an initial crisis response team or as an alternative to a higher level of care. The primary goal is to help stabilize the individual within their natural environment while engaging their support system and coordinating long-term care solutions.

Services are delivered in the individual's natural setting—such as their home or community—and may include:

  • Brief therapeutic and skill-building interventions

  • Engagement and integration of natural supports

  • Assistance with de-escalation and crisis resolution

  • Coordination of follow-up services and referrals

 

Community Stabilization can also connect individuals with co-occurring conditions—such as intellectual/developmental disabilities or substance use disorders—to specialized resources. Advocacy and care coordination are central to this service, helping clients and their families access appropriate community-based programs, benefits, and assistance for which they may be eligible.

This service is ideal for:

  1. Individuals awaiting entry into a follow-up service after an initial Mobile Crisis Response

  2. Clients transitioning from a higher level of care when immediate access to the next service is unavailable

  3. Those in need of crisis intervention as a diversion from inpatient or emergency care

Service Eligibility

Individuals may qualify for services if they have recently experienced a behavioral health crisis (within 72 hours of admission) or are transitioning from — or at risk of requiring — a higher level of care. Eligible individuals need short-term support to identify and engage in services that help maintain safety and stability within the community.

To qualify, individuals must meet at least one of the following conditions:

  • High Risk of Crisis Recurrence: There is a significant likelihood of recurring crises without immediate support.

  • Limited Resources or Support: The individual lacks the ability and/or resources to maintain safety and stability in the community until long-term services are available.

  • Transitioning from Other Services: The individual has recently completed alternative crisis services or treatment, no longer meets criteria for those programs, but continues to require stabilization support in the community.

 

Additionally, the individual must currently exhibit moderate to high behavioral or emotional needs that, without intervention, would interfere with functioning in one or more key areas of life, including:

  • Family relationships

  • Living situation

  • School performance

  • Social interactions

  • Work or community involvement
